ABSTRACT Objective: To assess the in-vitro effect of single applications of CPP-ACP pastes and different fluoridated solutions on the prevention of dental caries around orthodontic brackets. Material and Methods: Tooth/bracket sets (n=65) were immersed in artificial saliva (1h at 37ºC) and randomly subjected to single applications (100µL; 1min) of casein phosphopeptide-amorphous calcium phosphate (CPP-ACP emulsion), CPP-ACP with fluoride (CPP-ACPF emulsion), solutions of titanium tetrafluoride (TiF4) or sodium fluoride (NaF), or no treatment (CG). Multispecies biofilm (5 x 105 CFU/mL) was formed in the presence of 2% sucrose. After 24 h, the pH and the concentration of total soluble fluoride (TSF) were analyzed by culture medium. The presence of active white spot lesions (WSL) evaluated by macroscopic examination and the percent surface mineral loss (%SML) were analyzed. Also, the topography of enamel was detected by analysis of scanning electron microscopy (SEM). The data was assessed by chi-square, Kruskal-Wallis, and Mann-Whitney tests (p < 0.05). Results: Fluoride-containing compounds led to a smaller pH reduction than did CPP-ACP and CG (p<0.05). There was difference in TSF between the groups (p<0.05), denoted as TiF4> NaF > CPP-ACPF > CPP-ACP > CG. Regarding the presence of WSL and %SML, the NaF group obtained lower values (p<0.05), while TiF4 and CPP-ACPF were similar (p>0.05). SEM demonstrated that fluoride-free groups had a larger surface dissolution. Conclusion: Fluoridated groups including solutions and CPP-ACPF were more effective than CPP-ACP in reducing enamel demineralization around orthodontic brackets after a single application.


RESUMO Objetivo: Avaliar in-vitro o efeito de uma aplicação única de cremes dentais de CPP-ACP e diferentes soluções fluoretadas na prevenção da cárie dentária ao redor de braquetes ortodônticos Material e Métodos: O conjunto dentes/braquetes (n=65) foi imerso em saliva artificial (1h em 37°C) randomizado e submetido a tratamento único (100µL; 1 min) de emulsão de fosfopeptídeo de caseína-fosfato de cálcio amorfo (CPP-ACP) e CPP-ACP associado ao flúor (CPP-ACPF); soluções de tetrafluoreto de titânio (TiF4) e fluoreto de sódio (NaF); e ausência de tratamento (GC). Biofilmes multiespécie (5 x 105 CFU/mL) foram formados na presença de sacarose a 2%. Após 24h, o pH e a concentração de fluoreto solúvel total (FST) foram analisados pelo meio de cultura. Foram avaliadas a presença de lesões de mancha branca (LMB), por meio da análise de macroscopia visual, e a porcentagem de perda de dureza (%PD). Também foi verificada a topografia do esmalte, usando microscopia eletrônica de varredura (MEV). Os dados foram analisados pelos testes Qui-quadrado, Kruskal-Wallis e Mann-Whitney (p < 0,05). Resultados: Os compostos contendo flúor levaram a uma redução do pH menor do que o CPP-ACP e GC (p<0,05). Houve diferença no FST entre os grupos (p <0,05), sendo TiF4> NaF > CPP-ACPF > CPP-ACP > GC. Quanto à presença de LMB e à %PD, o grupo NaF obteve os menores valores (p<0,05), enquanto TiF4 e CPP-ACPF foram semelhantes (p> 0,05). A MEV demonstrou que os grupos sem flúor tiveram uma dissolução superficial maior. Conclusão: Os grupos fluoretados, incluindo soluções e CPP-ACPF, foram mais eficazes do que o CPP-ACP sem flúor na redução da desmineralização do esmalte ao redor dos braquetes ortodônticos após uma única aplicação.

Abstract Objective: To evaluate the efficacy of silver diamine fluoride (SDF) in arresting dentin caries lesions when applied under different concentrations and times. Material and Methods: Forty-two bovine blocks were selected and fixed in 24-well plates. Each well received a mixed bacterial inoculum added to the culture medium with 5% sucrose. The plates were incubated in microaerophilia (7 days) for caries formation, confirmed by micro-CT (M1). SDF was applied over the carious lesions for different times and concentrations (n=6): SDF 30% - immediate removal, 1 minute and 3 minutes; SDF 38%, - immediate removal, 1 minute and 3 minutes. The group without treatment was the control. Then, the samples were again scanned by micro-CT (M2) and submitted to a second cariogenic challenge for 21 days. Then, a final scan was performed (M3). Results: Mean pH at the culture medium and lesion depth were compared using Kruskal-Wallis and Wilcoxon tests. 38% SDF showed the lowest metabolic activity of the biofilm. All 38% groups and 30% 1 and 3 minutes did not show an increase in mean lesion depth comparing M3 with M1. However, only 30% 3 minutes and 38% 1 and 3 minutes showed a significant reduction of lesion depth. Conclusion: The minimum application time of 30% SDF to arrest dentin caries lesion was 1 minute, while 38% SDF arrested with application and immediate removal.

ABSTRACT

Objective: To evaluate the antimicrobial activity of antibiotic pastes used in lesion sterilization and tissue repair (LSTR) technique, through a novel membrane direct contact methodology against a multispecies biofilm and to establish appropriate dilutions for this method. Methods: CTZ (chloramphenicol, tetracycline, zinc oxide) and two formulations of 3Mix pastes (ciprofloxacin, metronidazole, and minocycline), 3Mix1 and 3Mix3, were evaluated with negative (0.9% saline) and positive (chlorhexidine 0.2%) control groups. Candida albicans and Enterococcus faecalis (24-hour) biofilms (n=10) grown on cellulose membranes were directly exposed to standardized amounts of fresh pastes and control solutions (n=2) for 24h. Membranes were immersed in 900 µl of saline solution, and seven serial dilutions were made for each sample. Plating for each dilution (n=2) was performed on culture media for microbial colony-forming unit (CFU) counting of total microorganisms, Candida spp. and Enterococcus spp. Aiming the comparison between groups, CFU quantification data were transformed into log10 CFU / mL and the Mann-Whitney test was applied (p<0.05). Results: Inhibition of CFU was observed for all pastes, with greatest effects for CTZ paste in medium selective for Candida spp. (p<0.001) and 3Mix1 in non-selective (p<0.000) and selective for Enterococcus spp. (p<0.004). Conclusion: The pastes showed antimicrobial activity against the tested multispecies biofilm, and the proposed direct contact methodology was efficient. Moreover, the dilutions used proved to be appropriate for this methodology.


Objetivo: Avaliar a atividade antimicrobiana de pastas antibióticas utilizadas na técnica Lesion Sterilization and Tissue Repair (LSTR), através de nova metodologia de contato direto com membrana contra um biofilme multiespécies e estabelecer diluições adequadas para avaliação. Métodos: CTZ (cloranfenicol, tetraciclina,óxido de zinco) e duas formulações de pastas 3Mix (Ciprofloxacina, Metronidazol e Minociclina), 3Mix1 e 3Mix3, foram avaliadas, além dos grupos controle, negativo (solução salina a 0,9%) e positivo (clorexidina 0,2%). Biofilmes de Candida albicans e Enterococcus faecalis cultivados sobre membranas de celulose (n=10) durante24 h foram expostos diretamente em contato com quantidades padronizadas de pastas frescas e controles (n = 2) por 24 h. As membranas foram imersas em 900µL de solução salina e sete diluições seriadas foram obtidas por amostra. O plaqueamento para cada diluição (n = 2) foi realizado em meios de cultura para microrganismos totais e seletivos para Candida spp. e Enterococcus spp. para contagem de unidades formadoras de colônias (UFC). Para comparação entre grupos, os dados da contagem de UFC foram convertidos em log10 UFC / mL e o teste Mann-Whitney foi aplicado (p<0,05). Resultados: Observou-se inibição de UFC para todas as pastas, maior para CTZ no meio seletivo para Candida (p<0,001)e 3Mix1 nos demais meios (p<0,004). Conclusão: Concluiu-se que as pastas apresentaram atividade antimicrobiana contra o biofilme multiespécies testado e que a nova metodologia de contato direto proposta foi eficiente. Além disso, as diluições utilizadas mostraram-se adequadas para essa metodologia.

ABSTRACT Purpose: the purpose of this research was to identify the sociodemographic and microbiological characteristics and antibiotic resistance rates of patients with diabetic foot infections, hospitalized in an emergency reference center. Methods: it was an observational and transversal study. The sociodemographic data were collected by direct interview with the patients. During the surgical procedures, specimens of tissue of the infected foot lesions were biopsied to be cultured, and for bacterial resistance analysis. Results: the sample consisted of 105 patients. The majority of patierns were men, over 50 years of age, married and with low educational level. There was bacterial growth in 95 of the 105 tissue cultures. In each positive culture only one germ was isolated. There was a high prevalence of germs of the Enterobacteriaceae family (51,5%). Gram-negative germs were isolated in 60% of cultures and the most individually isolated germs were the Gram-positive cocci, Staphylococcus aureus (20%) and Enterococcus faecalis (17,9%). Regarding antibiotic resistance rates, a high frequency of Staphylococcus aureus resistant to methicillin (63,0%) and to ciprofloxacin (55,5%) was found; additionally, 43,5% of the Gram-negative isolated germs were resistant to ciprofloxacin. Conclusions: the majority of patients were men, over 50 years of age, married and with low educational level. The most prevalent isolated germs from the infected foot lesions were Gram-negative bacteria, resistant to ciprofloxacin, and the individually most isolated germ was the methicillin resistant Staphylococcus aureus.


RESUMO Objetivo: identificar o perfil sociodemográfico, microbiológico e de resistência bacteriana em pacientes com pé diabético infectado. Métodos: tratou-se de estudo observacional, transversal que avaliou os perfis sóciodemográfico e microbiológico de pacientes portadores de pé diabético infectado internados em Pronto Socorro de referência. Os dados sociodemográficos foram coletados por meio de entrevista. Foram colhidos, durante os procedimentos cirúrgicos, fragmentos de tecidos das lesões podais infectadas para realização de cultura/antibiograma. Resultados: a amostra foi composta por 105 pacientes. O perfil sociodemográfico mais prevalente foi o de pacientes do sexo masculino, acima dos 50 anos, casados e com baixa escolaridade. Das 105 amostras de fragmentos de tecidos colhidos para realização de cultura e antibiograma, 95 foram positivas, com crescimento de um único germe em cada um dos exames. Houve predomínio de germes da família Enterobacteriaceae (51,5%). Germes Gram-negativos foram isolados em 60,0% das culturas e os espécimes mais isolados individualmente foram os cocos Gram-positivos, Staphylococcus aureus (20,0%) e Enterococcus faecalis (17,9%). Considerando-se os perfis de resistência bacteriana, verificou-se alta taxa de Staphylococcus aureus resistente à meticilina (63,0%) e à ciprofloxacino (55,5%); verificou-se, também, que 43,5% dos germes Gram-negativos eram resistentes à ciprofloxacino. Conclusões: o perfil sociodemográfico majoritário, foi o de homens, com mais de 50 anos e com baixa escolaridade. Concluímos que os germes mais prevalentes nas lesões podais dos pacientes diabéticos foram os Gram-negativos, resistentes ao ciprofloxacino e que o germe mais isolado individualmente foi o Staphylococcus aureus resistente à meticilina.

ABSTRACT

The World Health Organization (WHO) recommends as a weekly "target dose" of exercise 150 minutes of moderate exercise or 75 minutes of intense exercise. Public health policies have prioritized the practice of exercise as a strategy for disease prevention and health promotion, with health professionals as their main promoters. Objective: To assess the interaction between the amount of exercise per week and the knowledge about recommendations for fighting a sedentary lifestyle among health care professionals attending a congress of cardiology. Methods: Participants of the 2017 Rio de Janeiro Society of Cardiology Congress were interviewed. Knowledge about the World Health Organization (WHO) recommendations for fighting a sedentary lifestyle was assessed by asking participants the question: "How much weekly exercise is recommended by the WHO?" Responders were stratified by the weekly exercise load reported. A multivariate logistic model was created to determine independent predictors of knowledge. Results: A total of 426 participants were interviewed (45.5% men, median age 31 years, 37.8% physicians, 65.8% of the physicians were cardiologists). The overall knowledge level was 44.6%; 38.1%, 52.7% and 56.6% among non-physicians, non-cardiologists and cardiologists, respectively (p = 0.002). Of all participants, 21.8% were inactive, 15% were lightly active, 34.7% moderately active and 28.4% highly active, and the percentage of individuals who gave a correct answer to the question on exercise recommendations was 30.1%, 42%, 48% and 52.9% respectively (p < 0.0001). In the multivariate analysis, being highly active (OR = 2.25, IC95%, 1.238 - 4.089), moderately active (OR = 1.93, IC 95% 1.105 - 3.39) and being a cardiologist (OR = 2.01, IC 95% 1.243 - 3,267) were predictors of knowledge. Conclusions: There was a linear association between exercise level and knowledge about the WHO recommendations on exercise. Policies to stimulate the practice of exercise among health professionals can positively impact campaigns for reducing sedentary lifestyle in the general population

ABSTRACT

Objective: To evaluate in vitro the effect of a red propolis ethanolic extract (RPE) in the prevention of growth of a cariogenic biofilm and its cytotoxic potential. Material and Methods: Minimum inhibitory and bactericidal concentrations (MIC and MBC) of RPE against Streptococcus mutans and Lactobacillus casei were determined. The cytotoxic potential of 0.4% RPE in oral fibroblasts was observed after 1, 3 and 5 min of contact. Cellulose membrane disks (13 mm, N=12) were used for biofilm formation (24 h) of S. mutans and L. casei, which were treated (1 min) with 0.4% RPE or 0.12% Chlorhexidine (CHX). The control group of biofilm formation was not submitted to any treatment. Serial dilutions were then made to evaluate microbial viability. Descriptive data analysis and, for microbial viability, Mann Whitney test were performed (p≤0.05). Results: RPE showed similar MIC and MBC (4.46 mg/mL) against S. mutans and, for L. casei, they were 8.92 mg/mL (MIC) and 17.85 mg/mL (MBC). CHX presented MIC and MBC <0.00002 mg/mL for S. mutans and 0.00047 mg/mL for L. casei. After 1, 3 and 5 min, the RPE exhibited, respectively, 69.38%, 43.91% and 40.36% of viable cells. The RPE (6.55) and CHX (6.87) presented similar efficacy to reduce the total number of viable bacteria (p>0.05). Regarding the total number of viable bacteria (Log10 CFU/mL), the RPE (6.55) and CHX (6.87) presented similar efficacy (p>0.05). Conclusion: Red propolis extract showed antibacterial activity against the tested strains, exhibited acceptable cytotoxicity and reduced the colonization of S. mutans and L. casei in a biofilm membrane model.

ABSTRACT

O aumento da expectativa de vida é crescente e, concomitante a isto, há a valorização de algumas patologias, que acompanham a senescência − dentre elas as demências. No Brasil, 54,15% dos pacientes com demência foram diagnosticados com doença de Alzheimer, que é um distúrbio neurodegenerativo. A terapia medicamentosa apresenta benefícios limitados, sendo importante uma busca de terapias adjuvantes, para melhorar a qualidade de vida e retardar a institucionalização. O objetivo deste estudo foi correlacionar os benefícios do exercício físico para o paciente com doença de Alzheimer, e a melhora na qualidade de vida e na evolução do doente. Foi realizada uma revisão bibliográfica de artigos indexados nas bases SciELO, LILACS e PubMed, publicados entre os anos de 2010 e 2016, utilizando os descritores "doença de Alzheimer" e "exercício físico", pesquisados em português e inglês. Foram considerados para análise artigos redigidos em língua portuguesa, inglesa e espanhola. Foram encontrados 38 artigos no PubMed, 5 no LILACS e 4 na SciELO, sendo 28 selecionados, por sua maior relevância ao tema. O exercício físico possui efeito neuroprotetor na função cognitiva, por provocar redistribuição do fluxo sanguíneo cerebral, ação antioxidante de enzimas de reparação e citocinas pró-inflamatórias, degradação de placas amiloides, aumento do metabolismo e da síntese de neurotransmissores, resultando em mudanças cerebrais benéficas − estrutural e funcionalmente. Exercícios físicos demonstraram bons resultados na preservação da função cognitiva e funcional desses pacientes. Porém, ainda são necessários mais estudos acerca do tema, pelo fato de as pesquisas existentes apresentarem algumas limitações, principalmente quanto à amostragem. Este tema apresenta alta relevância e impacto da doença de Alzheimer nos pacientes.(AU)


Increased life expectancy is growing and, with it, some pathologies accompanying senescence, such as dementia, are more valued. In Brazil, 54.15% of patients with dementia were diagnosed with Alzheimer's Disease (AD), which is a neurodegenerative disorder. Drug therapy has limited benefits, and a search of adjuvant therapies to improve the quality of life and delay institutionalization is important. The objective of this study was to correlate the benefits of physical exercise in patients with Alzheimer's Disease, and to improve the quality of life and evolution of these patients. Literature review of articles indexed in SciELO, Lilacs and PubMed databases, published between 2010-2016, using the key words "Alzheimer's disease" and "physical exercise" was performed. The articles written in Portuguese, English or Spanish were considered. Five articles were found in Lilacs, 38 in PubMed and 4 in Scielo, with 28 being selected due to their relation with the theme. Physical exercise has a neuroprotective effect on cognitive function by causing redistribution of cerebral blood flow, antioxidant action of repair enzymes, and pro-inflammatory cytokines, degradation of amyloid plaques, increased metabolism and synthesis of neurotransmitters, resulting in beneficial structural and functional brain changes. Physical exercises have shown good results in these patients' cognitive and functional preservation and/or improvement. However further studies about the subject are necessary because the existing studies have some limitations, mainly regarding sampling. This topic will have high relevance and impact of Alzheimer's Disease on patients.(AU)

Na assistência à mulher lactante percebe-se a necessidade de aperfeiçoamento e individualização do cuidado de enfermagem e a utilização de um sistema de linguagem uniformizada favorece a troca de informação e a padronização da comunicação entre os enfermeiros. A Classificação Internacional para a Prática de Enfermagem (CIPE®) é um instrumento tecnológico em desenvolvimento no cenário mundial, que permite ao enfermeiro mais autonomia e documentar sistematicamente sua prática assistencial. Trata-se de um estudo exploratório-descritivo desenvolvido com o objetivo de elaborar as intervenções de enfermagem para a amamentação após os seis meses de vida, tendo como base as diretrizes do Conselho Internacional de Enfermeiras e os termos do Modelo de Sete Eixos da CIPE® Versão 2.0, complementados com os da literatura da área. As combinações de termos resultaram em 54 intervenções de enfermagem. Conclui-se que a CIPE® constitui-se um ótimo instrumento para a implantação de uma linguagem unificada, a fim de facilitar a comunicação e aperfeiçoar a prestação de cuidados às mulheres lactantes.


Assistance in the lactating woman realizes the need for improvement and individualization of nursing care, and the use of a standardized language system favors the exchange of information and standardization of communication between nurses. The International Classification for Nursing Practice (ICNP®) is a technological tool for development on the world stage, which gives nurses more autonomy and systematically documents their care practice. This is a descriptive exploratory study developed with the goal of developing nursing interventions for breastfeeding after six months of life, based on the guidelines of the International Council of Nurses and the terms of the Model Seven axes of ICNP® version 2.0, supplemented with the literature. Combinations of terms resulted in 54 nursing interventions. We conclude that the ICNP® constitutes a great tool for the implementation of a unified language, in order to facilitate communication and improve the delivery of care to women who are breastfeeding.


Asistencia en la mujer en período de lactancia se da cuenta de la necesidad de mejora y la individualización de los cuidados de enfermería y el uso de un sistema de lenguaje normalizado favorece el intercambio de información y la normalización de la comunicación entre las enfermeras. La Clasificación Internacional para la Práctica de Enfermería (CIPE®) es una herramienta tecnológica para el desarrollo en el escenario mundial, lo que permite a las enfermeras más autonomía y documentar sistemáticamente su práctica asistencial. Se trata de un estudio exploratorio descriptivo, desarrollado con el objetivo de desarrollar intervenciones de enfermería para la lactancia materna a los seis meses de vida, con base en las directrices del Consejo Internacional de Enfermeras y los términos del modelo de siete ejes de CIPE® Versión 2.0, complementado con la literatura. Las combinaciones de términos produjeron 54 intervenciones de enfermería. Llegamos a la conclusión de que la CIPE® constituye una gran herramienta para la implementación de un lenguaje unificado con el fin de facilitar la comunicación y mejorar la prestación de la atención a las mujeres que están amamantando.
